Appointments and Forms

Our office hours are Monday-Friday 8:00 AM to 4:00 PM.
Our phones are set to an answering service between 12:00 PM and 1:00 PM for lunch.

Please give 24 hours notice If you need to cancel your appointment. For urgent appointment requests, please call our office.

Once you have made your appointment, you can click on the links below to download the New Patient Forms. You may print and complete the patient forms prior to your arrival. Doing so will speed up the new patient registration process.

Medical Records

Patients may access a summary of their medical records, along with any labs, imaging, and documents that their provider has made available at any time through our Healow Patient Portal. Please see our guide to setting up a patient portal/accessing Healow TeleHealth visits here.

If you require medical records to be transferred and cannot access your patient portal, please fill out our medical records release:

Completed forms can be brought to our office, mailed in, or faxed in. Record requests typically take between two weeks and one month to fulfill. If you require them sooner than that, or require records to be returned in a special way, please include that on the request form or give us a call.

Insurance

Please bring a photo ID and current insurance card to your appointments.  We can work with you and your insurance company, as well as offer payment plans.

We are contracted with the following insurance companies:

  • Aetna

  • Blue Cross Blue Shield of Alaska

  • Federal Blue Cross

  • Premera Blue Cross

  • Medicaid

  • Medicare

  • Tricare Select

  • MultiPlan (PHCS, Beech Street, and HealthEOS)
